Charge Conference Forms

 

Instructions:
 

Before proceeding please read all directions carefully.

You do not need a password to use the forms. As you fill out the forms, no data is saved for future completion.  You must complete the form in one sitting. You just select the form you wish to fill out, type in your information, and click on submit. After submitting the form, the form will be emailed to your district, emailed to the address filled out on the form, and displayed in the webbrowser so you can print it out. If you would rather fill out the form by hand, pdf versions of the forms are available. Some things to remember. Make sure you select which district you are part of before submitting the form. If you do not do this, the district will not get a copy of your form by email. A copy of the form will be emailed to the email address that is entered at the bottom of the form. If you do not see the email after submitting it, check you junk mail and see if it is there. If you have a problem, call the conference office and ask for extension 3102.

The only form that cannot be filled out like this is the Annual Parsonage Inspection Report (form 6). This form has to be downloaded as a PDF and filled out by hand. To continue to the forms page, click below.
